American health officials are raising the alarm as cases of bacterial meningitis have been steadily increasing over the past few years. While cases of meningitis had been on a steady downward course prior to the COVID-19 pandemic, with just 371 cases of meningitis reported in 2019, there were 503 confirmed cases in 2024. While still rare in the U.S., the rise has prompted public health experts to remind citizens about the importance of vaccination and early detection.
Understanding Meningitis
Meningitis is a serious infection of the membranes surrounding the brain and spinal cord. Inflammation of the meninges (the three protective membranes that cover and protect the brain and spinal cord) puts pressure on the spinal cord and brain, increasing intracranial pressure (the pressure exerted by the fluids inside the skull and disrupting blood flow). Meningitis can be caused by viruses, bacteria, fungi, injuries, cancer, and drug abuse. Viral meningitis is more common but usually less severe. Viral meningitis typically presents itself with a sudden onset of fever, severe headache, stiff neck, and fatigue—usually lasting 7–10 days. It is typically spread through close contact with an infected person (kissing, coughing, sneezing, etc.) or by touching contaminated surfaces. Bacterial meningitis can lead to permanent complications in one out of five cases, including hearing loss, brain damage, seizures, kidney damage, limb amputation, and cognitive or behavioral disabilities (memory loss, anxiety, depression, learning disabilities, slowed cognitive processing, etc.).
Why Are Health Officials Concerned About NYC?
Although bacterial meningitis remains rare in NYC, health officials are concerned about a steady upward trend: 15 reported cases in 2021, 19 in 2022, 28 in 2023, and 11 as of April 2, 2024, including one death. Public health officials note that while the reasons for this increase are not fully understood, certain types of the meningitis bacteria, like serogroup Y, have been responsible for more infections recently. This is due to serogroup Y having the ability to spread more easily in close-contact settings.
Vaccination is Key
Vaccines are a critical line of defense against meningococcal disease, a severe form of bacterial meningitis. Meningococcal disease is caused by certain types of bacteria that spread more easily than other forms of bacterial meningitis, causing it to progress very quickly, sometimes within 24–48 hours of symptom onset. Recommended vaccines include MenACWY for preteens (ages 11–12) with a booster at age 16 and MenB for certain groups or during outbreaks. Health authorities urge residents at higher risk—such as immunocompromised people or people living in communal housing—to ensure their vaccines are up to date (even adults need to get a meningococcal vaccine every five years if they are high risk). Vaccines are widely available at clinics, pharmacies, and doctors’ offices, and are often covered by insurance. If you are an NYC student and are concerned that you do not have this vaccine, don’t worry; in NYC, meningitis vaccinations (MenACWY) are required to enter school from the 7th to the 12th grades. Specifically, students entering 7th grade need one dose, and a second dose (booster) is required for entry into 12th grade, typically at age 16 or older.
However, if you or anyone you know has not received the vaccine, they may be at higher risk of infection. That’s why it’s important to be aware of the signs and symptoms of meningitis and to act quickly if they appear.
Act Fast if Symptoms Appear
If you suspect that you or someone you know may have bacterial meningitis, act fast by seeking emergency medical care immediately when recognizing early symptoms, because untreated meningitis can be fatal within 24 to 48 hours. Symptoms to watch for include fever, severe headache, stiff neck, sensitivity to bright light, vomiting, unusual drowsiness, confusion, seizures, and a distinctive rash that does not fade unless glass is rolled over it. The glass test is a simple, at-home way to check for a meningitis-related rash. Press a clear glass against the skin; if red, purple, or brown spots do not fade (non-blanching), it could indicate meningococcal infection and requires immediate medical attention. The rash often starts as tiny pinpricks and can develop into larger, bruised-looking patches, but it may not appear in all cases. Even without a rash, other serious symptoms like high fever, stiff neck, severe headache, vomiting, or confusion are medical emergencies and require urgent care.
Treatment
Once at the hospital, patients are usually treated with intravenous (IV) antibiotics to fight the infection, fluids to prevent dehydration, and medications to control fever, pain, or seizures. In severe cases, patients may require oxygen therapy or intensive care to support vital functions.
Preventative Measures
Preventive antibiotics (prophylaxis) are a separate measure for close contacts of an infected person. These medications are given before anyone develops the infection to reduce their risk of getting meningococcal disease. They do not treat someone who is already sick, so they are not a substitute for urgent medical care if symptoms appear.
While bacterial meningitis is rare, its consequences can be devastating. With rising cases in New York City, experts emphasize vaccination, vigilance, and rapid medical attention as the best ways to protect against this potentially life-threatening infection. Schools in particular are places where students spend hours together in classrooms, cafeterias, gyms, and on sports teams—close-contact environments where infections can spread more easily. Even a single case of bacterial meningitis in a school could put classmates, teachers, and staff at risk. That’s why staying aware of symptoms, keeping vaccines up to date, and following preventive measures (like not sharing drinks or utensils) is especially important for students.
